Summary of Changes from Previous Version

5 May 2021 – System Administrator Fix – updated the placeholder fo the Biosafety and Radiation Safety Committee from the expired Division-based record to the Committee-based record.


20 March 2019 – removed embedded URL and replaced with associated information placeholder URLs.


13 November 2017 – Document amended to update hyperlink to the Work Health and Safety Regulation 2011 to the current Work Health and Safety Regulation 2017.


 


Cl 29 was amended to change the point at which a relevant Dean or Director co-signs a document for submission to NSW Health Pharmaceutical Services on application for authority to possess drugs of addiction or prohibited substances. The relevant Dean or Director was previously required to co-sign the Application for Authority to Possess Drugs of Addiction or Prohibited Substances for the Purpose of Research, Analysis, or Instruction form, but as this form is now completed electronically, it does not allow for a co-signature. Application to Pharmaceutical Services also requires the submission of a cover letter. Acquiring the signature of the relevant Dean or Director on this cover letter will ensure compliance with the requirements of NSW Health Pharmaceutical Services, and make certain that relevant Deans or Directors are aware of the use of such substances in areas under their control.
